AARF Adoption Contract

 

Congratulations on the adoption of your new family member and thank you for choosing adoption!  In accordance with this adoption, Animal Allies Rescue Foundation, Inc. (AARF) requires all of the following is acknowledged, understood, and agreed upon:

 

1. Adopted animal is spayed/neutered, current on vaccinations (DHLPP/FVRCP/Bordatella and rabies if at least 3-4 months of age), microchipped, heartworm 4DX tested (Ehrlichia/Anaplasmosis/Lyme/ Heartworm) and treated if positive for canines, and FIV/FeLV tested for felines. If applicable, the animal has also been dewormed and fecal-tested. Adopter is responsible for any follow-up vet care to include, booster shots (DHLPP/FVRCP), heartworm preventative, etc. AARF has the right to obtain proof of vaccinations and spay neuter upon request*.

*If kitten or puppy is under age for alteration and/or rabies vaccine, or the dog is heartworm positive, an adoption addendum will be included which is considered part of this adoption contract.

2. AARF prohibits declawing, cosmetic surgery, tail docking, and ear cropping. Adopter agrees to abide by all of the above. Should adopter fail to abide by this request, AARF reserves the right to enter personal property and redeem the animal. Should medical expenses arise due to any of the above type of surgeries, the adopter will be liable to pay for up to $2,500 in medical expense compensation to AARF.

3. Adopted animal will be slowly introduced to all new people, situations and other animals as the animal’s background is not known. This process may take several months.  If further advice is needed, please consult with someone at AARF for tips on proper introductions.

4. AARF makes no guarantees or promises as to animal’s health, temperament, or behavior or length of adjustment time. 

5. AARF is hereby released and held harmless from any and all liability as to any injuries or damages resulting from this animal, relating to the animal, or arising out of the animal’s behavior. 

6. Adopted animal will never be left unattended outside on a tether or chain 24/7. If AARF is notified of excessive outdoor habitation, AARF reserves the right to enter personal property and redeem the animal. Should medical expenses arise due to this neglect, the adopter will be liable to pay for up to $2,500 in medical expense compensation to AARF.

7. AARF requires a new collar, ID tag and leash to be purchased by the adopter before adoption of a dog; and for cats, it is required to have a “break-away” collar with ID tag. It is also strongly suggested to have an appropriate size crate readily available for separating the animal and supplying a safe haven, at least during the initial adjustment period.

8. Proper licensing as required by the adopter’s local ordinance city/county/state will be acquired by adopter and worn by the animal at all times.

9. Adopted animal is microchipped and automatically registered with adopter’s contact information.   Adopter agrees to keep contact information current (especially important should address/phone change) during lifetime of adopted animal. Adopter will be notified via email upon successful microchip registration and will be provided with instructions on how to maintain personal contact information via www.24PetWatch.com.  

10. Dependent on meeting eligibility requirements: Dogs 8 weeks to 10 years of age and cats 8 weeks to 12 years of age, adopted animal comes with an offer for 30 day trial of FREE pet insurance, however the adopter is under no obligation to accept this offer. If the adopter decides to accept, the instructions on how to activate this trial will be sent via email from 24PetWatch. This offer is not currently available to residents of CA, PA or RI.

11. Should the need arise to surrender/return/rehome the adopted animal, adopter promises to contact AARF immediately and return the adopted animal to AARF. AARF will then place the animal in a suitable new home. Under no circumstances will the animal be taken to a shelter.

12. The adoption donation is non-refundable. The donation is not a fee or sale price. Rather, the donation is for the purpose of reimbursing AARF for any and all past or future medical expenses related to the adopted animal or other rescue animals in AARF’s care, including spay/neuter, vaccinations, or any other expenses incurred to prepare the animal for adoption.

13. Adopter agrees to take animal to a licensed Veterinarian for examination within two weeks of receiving an email from AARF that will include all medical paperwork, copy of this contract and microchip information. AARF does not knowingly adopt out sick animals and is not responsible to pay veterinary expenses once this contract is complete regardless of when the animal became ill.  Adopter understands that rescue animals can have undetected illnesses and takes full responsibility for the animal’s medical care.
